Alice's Place, LLC is seeking a dependable, compassionate, and professional Assessment Nurse to join our Structured Family Caregiving (SFC) team.
Position Responsibilities
Position Responsibilities
The Assessment Nurse will:
- Complete initial and ongoing nursing assessments for Structured Family Caregiving participants
- Conduct in-home visits as assigned
- Assess participants' health status, functional needs, medications, and overall care needs
- Complete required nursing documentation accurately and on time
- Identify changes in condition and communicate concerns to the appropriate care team
- Provide caregiver education and support when needed
- Maintain participant confidentiality and follow HIPAA requirements
- Follow Alice's Place policies and applicable Medicaid/program requirements
- Travel to assigned participant homes throughout designated service areas
Qualifications
- Current Georgia nursing license
- RN or LPN with appropriate experience and qualifications for assigned assessments
- Experience working with the elderly and/or individuals with disabilities preferred
- Strong assessment and documentation skills
- Dependable transportation
- Valid driver's license and automobile insurance
- Excellent communication and organizational skills
- Must be dependable and able to complete documentation within required timeframes
- Home health, Medicaid waiver, community-based services, or long-term care experience is a plus

Compensation: $65.00 - $65.00 per day
Alice’s Place Adult Day Care is a fully licensed and insured adult day care and all of our employees are extremely understanding, skilled, and experienced in caring for the elderly. Our center is managed by a Registered Nurse/Licensed Nursing Home Administrator with more than 33 years of geriatric experience.
